Reproducible in Lucid/10.04 since 2.6.32-9.13
Not reproducible in Karmic/9.10 (note: linux-backports
Periodically my wireless device (ar9170-based, usb, 0846:9010 NetGear, Inc. WNDA3100 802.11n) spews phy2 errors in the kernel ring buffer (easily seen in dmesg output) after which I must remove it and reinsert it for it to reassociate with the wireless access point and obtain a dhcp lease.
This symptom occurs regardless of access point (manufacturer, open/wep/wpa/wpa2) configuration. I am able to reproduce this symptom on Cisco, Netgear, D-Link access points both with and without stock manufacturer firmwares.

I'd like to add that I see the same with my NetGear, Inc. WN111 v2 802.11n USB dongle (came packaged with the DG2100N ADSL router). This device uses the same driver and firmware (apparently -- since I found this bug via google).
If I can provide more information than just a "me too", please let me know.
I'm also seeing it on 10.04, kernel 2.6.32-22-generic
