Ah, your dmesg is full with errors like
[4296493.949000] FAT: Directory bread(block 39156) failed
[4296493.949000] 6:0:0:0: rejecting I/O to dead device
and then you end up with the same device being re-discovered as sdb, sdc, etc. |
