Bug Description

Bluetooth turns dead after a couple times enable/disable switches, leaving following error messages in dmesg only:

Not necessarily put bluetooth in a unresponsive state, but it does sometimes fail to reply all USB URBs and become completely dead unless a power cycle is executed.

Tested with the latest firmware from korg, commit 27a36893, REL17064. Also reproducible on 5.9 mainline kernel.
