Bug Description

Lubuntu groovy QA-test (LIVE) on
- dell [optiplex] 755 (c2d-e6850, 5gb, amd/ati radeon rv516/x1300/x1550)

** Background

I've been noticing this for awhile (was noticed first on Xubuntu), eg.

"xubuntu groovy daily; I've had a number (3) of boxes boot with sound muted; this isn't intentional is it?"

** What occurred this boot

Note this is not the initial boot of this box, it was booted twice with same results.

Sound is muted, I can't get sound working using the PANEL volume control.

I have to start `pavucontrol-qt` using the menu, go to the OUTPUT DEVICES tab, and click the 'speaker' button to enable sound.

** What I expect to occur

I'd expect sound to be enabled by default.

** Further comments

- this is minor, once fixed it's gone, also not all users will need sound & can be easily fixed as outlined. Most QA installs I don't re-use, but I do on occasion in support, I can't recall muted-sound reappearing (though most times I'd not use audio)
- it appears to be box specific, and occurring only on some older boxes
- this was filed with Lubuntu, but as initially noticed on Xubuntu (I've explored it before, but comments only made on IRC)
- filed on kernel but could be pulseaudio, alsa or other?

identical issue on today's (2020-10-17) on
- dell [optiplex] 755 (c2d-e6850, 5gb, amd/ati radeon rv516/x1300/x1550)
and
- lenovo thinkpad x201 (i5-m520, 4gb, i915)

(alas didn't think to test many boxes with this today)

firefox; stream news in window, maximized, f11 & fullscreen; alas it was silent. I needed to go to pavucontrol-qt and toggle output-device to ON (also increase volume, but I'm ignoring that)

I had to unmute, did it via middle-click?? on panel, though I find the pavucontrol-qt toggle in output-device easier (maybe it's just me)

To me sound shouldn't be MUTED by default; but my opinion
