This first occurred on Ubuntu 18.04, and keeps happening on 19.04 (which I had updated to in the hopes of fixing the issue that way). The only escape I've found from this state is via magic SysRq. It can be triggered reliably by waiting for the screensaver to kick in (e.g. overnight).
