Did this issue start happening after an update/upgrade? Was there a prior kernel version where you were not having this particular problem?

Would it be possible for you to test the latest upstream kernel? Refer to https://wiki.ubuntu.com/KernelMainlineBuilds . Please test the latest v4.16 kernel[0].

If this bug is fixed in the mainline kernel, please add the following tag 'kernel-fixed-upstream'.

If the mainline kernel does not fix this bug, please add the tag: 'kernel-bug-exists-upstream'.

Once testing of the upstream kernel is complete, please mark this bug as "Confirmed".

Thanks in advance.

As told in the description, with kernel 4.4.0-112-generic the outputs are recognized and work fine.
The next kernel I installed was 4.4.0-116-generic with which the bug appears. The bug is not fixed in v4.16-rc4.

Make sure your system is up to date.
$ sudo apt update && sudo apt -y dist-upgrade

View the nvidia module name/version:

$ dkms list

Rebuild your nvidia module
$ sudo dkms remove ${nvidia-module-name}/${nvidia-module-version} -k 4.4.0-116-generic
$ sudi dkms install ${nvidia-module-name}/${nvidia-module-version} -k 4.4.0-116-generic

reboot and test.

I used "dkms status" to get the required information about the module name (nvidia-340) and version (340.104)

After rebuilding and rebooting at least HDMI works with 4.4.0-116-generic
