Bug Description
I think this problem has something to do with either insufficient space in the /boot partition or some (improper) memory the system is keeping of its prior kernels. But I haven't time right now to try all of the advice below.

On a more general note, I'm running 14.04 now. Is it possible to directly *update* to 16.04 without intermediate steps, like 14.04 -> 14.06 -> ...-> 16.04. In my opinion your documentation is not as clear about this matter as it could and should be.
But without becoming too critical,
I'll leave it now.
