Dell Bluetooth card not working except enable/disable

Bug Description
After install this bluetooth card, it can be enabled and disabled, but blue manager always claims "Bluetooth is disabled"
usb-devices omits this card, even though it appears in lsusb.
Bus 003 Device 003: ID 413c:8141 Dell Computer Corp.
This has been a problem since before 14.04, and 16.04 has not yet solved it.
