Release notes:
Usage of SCSI LUNs on DS8870 Storage server with μCode Bundles 87.51.xx.0 (LMC 7.7.51.xx) via NPIV enabled zfcp adaptors causes detection issues. LP #1567602 In that case do not use NPIV enabled zfcp adaptors.

Scenario:
Using Installer 432.
No DASD devices, just two FCP CHPIDs with two LUNs each (configured for NPIV), provided via parmfile.
I expect the installer to probe and detect the LUNs automatically once I enable the FCP CHPIDs.

Repeated five times, I got different results each time. Detected LUNs vary between 2 and 4. One time 3 LUNs appear on SCSI1 and one on SCSI2 which looks especially odd to me.

Subsequent steps to create partitions and file systems in the installer fail.

Next, the installer warns that no disk drives were detected.

I checked this on a system (installer 445) with two NPIV enabled FCP devices that have 4 LUNS each. I only get the first of four LUNs per adaptor:

It looks like the LUN scan is being terminated.
Kernel parms with: scsi_mod.scsi_logging_level=4605 zfcp.dbflevel=6 zfcp.dbfsize=100

Re-testing with installer version 445 + udeb packages from 2016-04-07 s390-dasd 0.0.36ubuntu1, s390-zfcp 1.0.2ubuntu1, multipath-udeb 0.5.0+git1.656f8865-5ubuntu2, disk-detect 1.117ubuntu1

Some observations:
1. With one path only, the scanning/detection of all LUNs worked deterministically and correctly (including the final re-IPL after successful installation).
2. With two paths, but without the parmfile option 'disk-detect/multipath/enable=true', the LUNs were all detected, but once for every path. I did not want to continue installation that way.
3. With two paths and with the option 'disk-detect/multipath/enable=true', the multipath program was loaded and multipathing was set up correctly. ...
4. ... installation completed fine then...
5. .... but the final re-IPL after successful installation failed. Attaching last console messages of the IPL:

Meanwhile, with kernel 4.4.0-18 and installler version 447, the situation is different:
While the problem originally occured in installer, is happens now, when the target system is being IPLed.

The "switch" (to avoid the word "reason") is the kernel module scsi_dh_alua.
When scsi_dh_alua is loaded, the auto LUN scanning upon activating a zfcp device with NPIV LUNs causes that not all LUNs are detected. I assume, that not the kernel module itself is the problem, but t seems to trigger the problem.

A very easy way to reproduce this is as follows:
have a system installed with kernel 4.4.0-18 (almost GA). Have two NPIV enabled zfcp adaptors (on different PCHIDs) that can access the same (few) LUNs each. Then:

When a zfcp attached SCSI LUN is used for installation, (almost?) all scsi_dh modules are loaded upon activation of the zfcp device. This causes some LUNs not to be detected.
If I get acces to that system an blacklist the scsi_dh_alua module, the IPL works as expected ( i.e. all the LUNS are detected properly).

Hello, I just wanted to add to Thorsten's comment: we are also still working on analyzing this bug. To the best of my knowledge, the ALUA device-handler is not to blame here, like Thorsten also already suggested.

After looking at debug-data collected during reproductions, it seems like a SCSI-Command sent by the ALUA device-handler gets lost and thus causes a timeout after one Minute. This then...

The problem could not be seen under Debian kernel 4.2.0-1-s390x #1 SMP Debian 4.2.6-3 (2015-12-06) s390x, but under Ubuntu Kernel 4.3.0-7 and newer kernels:
With Debian kernel 4.4.0-1-s390x #1 SMP Debian 4.4.6-1 (2016-03-17) (on a debian userspace) it also occurs.

So this is an upstream kernel regression with certain hardware configuration? I'm looking into scans as seen on our hardware, but it appears to be consistently discovered on our combinations of hardware, but will stress test that more.

Also w.r.t. multipath, disk-detect/multipath/enable is now enabled by default on Ubuntu on s390x, thus multipath installer codepath should be the one offered by the installer when the devices are scanned correctly. Moving this bug report to kernel package, as it seems to me this is a kernel issue and it's not the installer logic that is at fault.

The problem of LUNs not being properly discovered happened only on the following combination:
Storage Server DS8870 AND ( auto LUN scan when onlining an NPIV enabled device OR scripted parallelized addition of LUNs) AND recent kernels > 4.3 AND scsi_dh_alua is loaded.

I also tried to re-create this on one of the lpars we got. No luck either. However I was using the latest kernel which was uploaded just ... I think yesterday (4.4.0-21). Thorsten, maybe you can try that version, too. Just to be on the safe side.

The dmesg I get also shows that alua probing is done. So that sounds to be going through the exactly same sequence. We got NPIV enabled, too. Only here I also got dasd which we use to ipl from. Mainly speculating here but it feels a bit like maybe newer kernels might send out inquiry commands quicker or in parallel and something in the environment fails to cope with that and silently drops some requests...

I interpret that as having the same problem/sympton as I have. Correct?

I also tried that today with kernel 4.4.0-21, same problem. Then we got a uCode update on our machine, uCode now brand-new from 04/18. Problem persists.

And I involved a tester in U.S. to reproduce this on their machine. Waiting for results.

I saw these problems today also on another machine / switch / DS8870 combination:
DS8870 uCode was 5.7.51.1009, which is also the newer stream.
Obviously the older stream 5.7.41.xx didn't show these problems.

At least the normal installation with non NPIV enabled SCSI LUNs on our DS8870 worked.
If I use NPIV enabled LUNs AND add the kernel parameter zfcp.allow_lun_scan=0 AND add the LUNs manually, it fails, if I have more than one LUN.
If I have NPIV enabled LUNs on DS8870 storage with microcode bundle 87.51.xx.x and use the LUN detection via NPIV, the installation fails.

Hello Canonical,
after talking with different folks here and trying to figure out why this
fails, we found the likely bug here, and its in the kernel after all.
When scanning the SCSI LUNs behind a found FCP remote port (during activation
of a FCP device) linux will also try to attach a device-handler to each found
LUN - with our storages it will try to attach the ALUA device handler
(scsi_dh_alua). In parallel to attaching the DH it will also activate the
scsi-disk driver for the found LUN. Those two will run in parallel. Schematic
this looks roughly like this (I hope this ASCII-Art survives in the Bugzilla):

Now it happens that sd_spinup_disk() sends the first command (a TUR) that is
not an INQURY or REPORT LUNS in parallel to the INQURY that is send by
alua_vpd_inquiry().
This TUR will raise a Check Condition with Sense Unit Attention because of a
reset in the storage - this is normal if the LUN was just attached the first
time after the remote port open (SAM-4, 5.14).
And because the storage has a default value of QErr=01b set in the Control
Mode Page (SPC-4, 7.5.8) this raised Check Condition will cause an Abort of all
running command in the same task set (same I_T nexus; SAM-4, 5.6).
But for this Abort no status will be returned for the affected commands in
the same I_T nexus as the command that received the Check Condition (also
SPC-4, 7.5.8).
So in essence the storage just forgets all other commands, other than the
first TUR, and that gets the Unit Attention (all other commands send before the
TUR are either INQURY or REPORT LUNS, and those will never raise a Unit
Attention; SAM-4, 5.14).

This is why the INQURY send by alua_vpd_inquiry() will ultimately time out
after one minute. And this will abort the whole LUN scan in
scsi_report_lun_scan().

Ofc this depends on specific timings - when arrives the INQURY and when the
TUR. But like written in this report, we have seen this in multiple completely
independent setups.

I looked at changes made by the maintainer of scsi_dh_alua upstream since
release of 4.4 and there has been some very substantial changes (recent changes
were titled "ALUA device handler update, part II" on LKML). And those changes
mitigate this problem. They change the timing of commands sent in scsi_dh_alua
and remove the send of own INQURYs completely (it uses ...

Thank you for your insights. Is it possible to determine a scope of affected systems out there? So far a specific config is required to trigger this bug it seems, but I'm not sure how common such a configuration is. Extensive back-ports and reworks of the kernel are out of the question. However, we do have LTS hardware enablement kernel stacks - https://wiki.ubuntu.com/Kernel/LTSEnablementStack#Kernel.2FSupport.A16.04.x_Ubuntu_Kernel_Support in essence, some time after 16.10 release, the 16.10 kernels will be backported to 16.04, and 16.04.2 release installation media will use the updated kernel by default - v4.8 (current estimate for 16.10 kernel), such that affected systems will be able to use much newer kernel during installation and regular runtime eventually in the future (current approximate timing February 2017).

Is it at all possible to test this issue with mainline kernels? and/or I guess we can wait for 16.10 to get upgraded kernel and retest on 16.10 to see if this issue is resolved with a newer kernel.

Hi Dimitri,

any customer using a zEC12, z13 with zfcp Adaptor connected via FC Switch to DS8000 any model having uCode Level 7.5 or newer might be affected. V7000 Storage is not affected, AFAICS today.
So, this is quite a big audience.
Considering that the kernel of 14.04/14.04.1 is supported until April 2021, I think that during this support timeframe customers will definitely hit this bug. We saw it on various setups here.
Since that problem prevents customers from installing Ubuntu at all (unblacklisting scsi_dh_alua and rebuilding the initrd is IMO not a "supported workaround"), our project management might raise severity soon.
So, IMO best idea is to get in touch with the maintainer of scsi_dh_alua to retrieve some ideas, which of all the patches might be the ones that fix the problem in 4.4. - if you do not want to apply them all.
Well, finally it's up to you/Canonical.

At this point, this appears to be a linux upstream issue. Could you please escalate this issue to the IBM kernel team? I would assume it would be in your interest to cherrypick/backport fixes for this issue into the v4.4-stable tree, which would then be picked up by us for 16.04 - v4.4 kernel.

Customers will get blocked during installation, which is really severe. HWE kernel might be too late for that, and besides that, 16.04.1 stream will need ita anyway somewhen within the supprted time frame.

Since this is a bug in common code and not in code parts that had been contributed by IBM, I do not see our kernel team reponsible for that.

It's up to Canonical to put this on risk and force customers to use the HWE kernel (in which it is hopefully fixed), or to contact the scsi_dh_alua maintainer and backport the required fixes.

We should be working together, because we do not have the mainframe setup/config which reproduces / exhibits this bug. And possibly neither upstream maintainer. It is common code, but the bug is triggered on hardware config and platform that IBM ships.

Nevertheless, behaviour is slightly better, but does not fix the problem completely nor gets rid of the described symptoms:
I installed the above mentioned kernel manually, loaded scsi_dh_alua kernel module and set two zfcp devices online (which will auto-enable four LUNs each) via chccwdev -e 1904,1944. Result is:
Sometimes the LUN detection runs through until all four LUNs are detected, sometimes it terminates LUN detection at an earlier time. In most cases the correlation is as follows: for the first zfcp device, 0...2 LUNs are detected, for the second zfcp device of the 'chccwdev -e' sequence 3...4 LUNs are detected.

So: Yes, it helps something. But it ist not completely fixed (nor worked around).

very good. The diligent wins!!
With this kernel I could not reproduce the symptom as described in the bug report. I suggest to go ahead with this, and by the way, it is also needed in the installer, where the problem originally occured (since there the scsi_dh_alua module is loaded).
