Hang when iSCSI targets are removed

Bug Description

There is a problem (Hang) when iSCSI targets are removed in the 4.4.0 kernel. The problem looks like it spans from 3.18 to 4.5. There is a bug in scsi_scan.c (scsi_mod) and scsi_transport_fc. Emulex tested a fix from Johannes(SUSE) that fixed it in the 4.4.0 kernel but I think it is still waiting to get picked up in a stable kernle. This Hang can happen on any devloss and on driver unload.

There is a patch from Bart van Asche that others reported is needed in order to fix this issue. It's called "Separate target visibility from reaped state information"
