Broadcom Bluetooth [0a5c:216f] does not work on 3.16 / 3.19 kernel, sometimes it can see the device (headset) but it can't pair
with it.

It looks like this is another Bluetooth device that needs proprietary firmware, like 0a5c:21d7 (bug 1129865)

The driver (btusb) is ready in 14.04, but this device won't work unless a proper firmware is provided.

With the 3.13 series kernels, it is looking for the firmware to be /lib/firmware/fw-0a5c_216f.hcd
