USB modem cannot be used if removed then reattached

Bug Description

The problem is produced as follows:
* Attach the USB modem to the laptop
* Connect to the internet using the modem
* Disconnect from the internet
* Remove USB modem
* Attach USB modem

It doesn't get detected by the system, and I get kernel oops,

This time I got the following usb_modeswitch error, then when I removed/reattached the modem I got the kernel oops:

Also, when I decide to poweroff/reboot the laptop it doesn't completely turn off, and the plymouth animation goes forever, I just have to keep pressing the power button to force it to turn off.

This is not an usb_modeswitch problem, it is a usb core or hub driver problem.
It causes a device connect/disconnect/connect/disconnect loop not only for the 3G modem but also for the Samsung Android device:
