Would it be possible for you to test the latest upstream kernel? Refer to https://wiki.ubuntu.com/KernelMainlineBuilds . Please test the latest v3.9 kernel[0]. You will need to install both the linux-image and linux-image-extra .deb packages.

If this bug is fixed in the mainline kernel, please add the following tag 'kernel-fixed-upstream'.

If the mainline kernel does not fix this bug, please add the tag: 'kernel-bug-exists-upstream'.

If you are unable to test the mainline kernel, for example it will not boot, please add the tag: 'kernel-unable-to-test-upstream'.
Once testing of the upstream kernel is complete, please mark this bug as "Confirmed".

I'm not sure if this is the same bug, but the information may be useful. For me the camera and audio work, but if my machine is left to sleep for a 'while' (longer than a few minutes?) it seems to disappear - Cheese and Google Hangouts can't find it (can't find audio or video). Logging out does not fix the problem but rebooting does. I forgot to see if it was still in the output from lsusb before I rebooted. Looking back at various syslogs, I found the following - the first line occurs 6 times.

This should be fixed in the upstream 3.14 kernel. Does this still occur with Ubuntu 15.04 "Vivid Vervet"?
