Unbeknown to me, on first boot a kernel is extracted from the /boot partition and replaces the current one. This would explain why the image worked on first boot (with my own kernel) and failed on the second (with a previous kernel). The newest version of the image now extracts the correct kernel for use on subsequent reboots.
