Bug Description

Dell Latitude E6540 (CID 201306-13679) on 12.04.4

Steps to reproduce the bug:
1. switch the wifi slider
2. observe the system [a]
3. switch back the wifi slider
4. observe the system [b]

Expected results:
[a]: wireless indicator on desktop closed. "rfkill list" showed hardware block yes
[b]:wireless indicator on desktop closed. "rfkill list" showed hardware block no

Actual results:
the wireless indicator on desktop and "rfkill list" message don't change.

Dell Latitude E7440 (CID 201304-13408) on 12.04.4 has this issue.
It has Qualcomm Atheros AR9485 Wireless Network Adapter [168c:0032]

I found more information to reproduce this bug:

Steps to reproduce the bug:
1. make sure the slider is "ON"
2. power on the machine
3. rfkill list shows the hard block is "no"
4. connect to a wifi, make sure the wireless work by ping command
5. switch the slider to be "Off"
6. ping again to make sure the wifi work
7. reboot
8. rfkill list shows the hard block is "yes", network-manager says "wireless is disabled by hardware switch"
9. ping, now it fails (connect: Network is unreachable.
10. switch the slider to be "On"
11. now wireless of network-manager is back and ping is ok. rfkill list show the hard block is "no"

That is, the slider could only turn on the wifi but not turn off the wireless.

Verified with 12.04.5 on 201304-13408 (3.13.0-32), this issue does not exist
