It looks to me like the firmware for the driver is missing:

That is a linux-firmware issue.

Do you happen to know if the firmware is also missing for Precise, or have you only tried a Quantal install?

This worked fine on previous quantal images, as well as the precise image. I will be testing the current daily ARM builds this evening and will update if I run into the same bug again or not.

Still failing on today's daily image. Joseph, what would be useful for you at this point? Would you like me to find the last previous known image that this worked under? I'm not sure I used wireless in the past on images, besides the precise image. I do know precise works.

This is likely the result of me removing firmware that the 3.5 kernel no longer uses, e.g., "UBUNTU: Remove obsolete ti-connectivity firmware". However, the ti-omap4 kernel is still based on 3.4, so I'll restore it for now.

This bug was fixed in the package linux-firmware - 1.89

---------------
linux-firmware (1.89) quantal; urgency=low

  * Revert "UBUNTU: Remove obsolete ti-connectivity firmware"
This firmware is still in use by the 3.4 kernel in ti-omap4.
    -LP: #1032779
  * UBUNTU: Remove duplicate qlogic firmware
