Bluetooth broken on pi3 with version 11

Bug Description

Whoops! Removed one of the bluetooth firmwares too soon; need to add brcm/BCM43430A1.raspberrypi,3-model-b.hcd back in (until we can get bluez-firmware into restricted next cycle and then remove it again; see LP: #2058234 for context).

Looks like pi-bluetooth also requires updating to support the newer kernel side of things too (new tests in udev rules, rfkill support, etc.)
