table of contents empty for no reason

Bug Description
Yesterday I got a problem with TOC being empty. Finally I could resolve it be recovering my outline numbering as stated in a openoffice forum:
"Go to Tools>Outline Numbering and in the Numbering tab, set Heading x to each level x missing (1 to 3). Then update the ToC."

All the numbering down to level 4 had to be redefined.
I save in Word document format, but as I see it, the outline numbering is a generic office setting, really don't know why it got lost.
