Thank you for taking the time to report this bug and helping to make Ubuntu better. We are sorry that we do not always have the capacity to look at all reported bugs in a timely manner. There have been many changes in Ubuntu since that time you reported the bug and your problem may have been fixed with some of the updates. It would help us a lot if you could test it on a currently supported Ubuntu version. When you test it and it is still an issue, kindly upload the updated logs by running apport-collect 440944 and any other logs that are relevant for this particular issue.

Thank you for your response.
I still cannot reproduce your issue... Could you try to provide additional details in order to reproduce the problem? Type of document (maybe a (example) document that triggers the problem, if it is document specific). Exact steps to trigger the problem? etc?

The steps I follow are in the bug report...
the only thing I can say is that I do fullscreen with f11. NOT MAXIMIZE.
Is this what you're doing?

 1. open a document in okular
 2. make it fullscreen (f11)
 2a. leave it open.
 3. open another document in a new okular window

 4. see that it is opened with now toolbar or menubar

Confirmed, I was using the 'presentation mode' (which does not trigger this issue) as F11 did not work (shortcut seems to be <ctrl>-<shift>-F for my setup)
