I'm not able to reproduce this on Ubuntu artful:
$ sudo ipset add myset 192.162.242.129
$ sudo ipset list
Name: myset
Type: hash:ip
Revision: 4
Header: family inet hashsize 1024 maxelem 65536

What type of set are you trying to add the IP to, and on which Ubuntu release?
