Various icons need 16x16 size

Bug Description

The Add Bookmark Icon in Epiphany and other programs is too large. I notice that it is in the 24x24 size only, and menu icons need to have a 16x16 icon available.

The dialog-apply icon is another one that is required in a 16x16 size, as this screenshot of Liferea shows.
