Although a real fix for this issue could easily end in a complex code re-engineering, I think we actually can patch it up in a manner that this bug could be counted as a "Paper Cut" (http://blog.davebsd.com/2009/06/03/paper-cut/):
As TM wrote back in 2004 (!):
* "Perhaps a better default behavior is to tell the user the file doesn't exist or has been moved
and to give the user the option to create a new file of the same type."
* And, in case, remove the entry from the list.
