This has just happened with me. I had put the Windows line after the "## ## End Default Options ##" and before the "### END DEBIAN AUTOMAGIC KERNELS LIST" in menu.lst to allow for the Windows partition to show up first. Probably some script automatically replaced the lines with Ubuntu default options (since it has correctly kept the partition mapping), but if this is the case, it should be noted in the file itself, like "AUTOMATICALLY GENERATED - DO NOT EDIT BETWEEN THESE LINES".
Special attention should be paid to this, since the standard user does not have the knowledge to replace it, and if Ubuntu keeps doing things like these whenever it's updated, users will just drop Ubuntu out of fear of not having their computer working.
