Installing 22.04 to get rid of buggy 16.0 installation; fatal error happened. No idea why

Bug Description

Upgraded from 14.04 to 16.04 and most functions crashed and could not be recovered. Used usb boot option to Try Ubuntu first, then install 22.04 to wipe out bad 16.04 installation. Fatal error happened during install.
