when upgrading 16.04 - 16.10 grub bootloader would not install and crashed

Bug Description

When trying to do the upgrade from 16.04LS to 16.10 the installer for the grub bootloader crashed with errors and could not recover and advised me to send this bug report

You are installing Ubuntu in UEFI mode on a disk without an EFI-System-Partition.

Please explain in more detail, how you are upgrading your system. Started an installation medium (DVD or USB)?
