Copied icons in dash after application run

Bug Description

When you launch an application (as example firefox) using a pinned icon, a new icon is created at the bottom of the panel, and instead of a picture with this application, a gear icon is shown. Launching new instances of the application generates new icons next to the pinned one, and they also have a gear instead of the application icon.

Expected: there should be only one icon per instance of application on the panel. Icons grouped. Used icons of application.

Instead: we have source pinned icon with properly application image and new grouped icons with gear image as icon at the bottom of panel.

Curiously, I've seen this issue with several Qt-based applications (Quassel, Veusz, and OpenSCAD) under noble, as reported in LP: #2044382, but I've not seen it with Firefox (which isn't Qt-based to the best of my knowledge?).

It is not only Firefox, as example Jetbrains Intelli Idea too. Lot of apps
