I have install some packages like alarm-clock, espeak and others(so I can't find full path to make this bug works in other system configuration, but I can show all needed configs, etc.) And I always install all updates, that shows me my repository mirror. When I try to open gnome-volume-control applet on panel, or start it with terminal, I just see many terminal windows(or maybe just 3-4 random-active windows) - and it looks like Windows(R) DesktopJailers(worms,viruses, that controls your desktop). htop and system-monitor shows many "orca" processes, sometimes opened gnome-volume-control dialog shows many speech-dispatcher processes and I hear voice.

Screenshots with opened System-monitor. All desktop was attacked with terminals and nothing was useful to destroy it - orca processes cannot be killed and I can just hit ctrl+alt+backspace for X restart

Well that didn't do anything LOL!

The first screenshot shows Orca crashing clearly but I can't duplicate the issue, are you able to test the 11.10 release to see if this still affects you? Also are you able to list the steps required to get the setup exactly like yours? Which programs you install and setup and how you set them up etc?

It is just 1-2 month after Ubuntu 11.04 installation but I can't tell all that I made to crash Orca - so many packages I installed at one time. But I can show all installed software:
