This came within the last 24h's updates, I am pretty sure. There was an upgraded gnome-icon-theme 2.13.5.1-0ubuntu1 yesterday, however, I think the icons were correct after that upgrade. Maybe a cache issue?

That's because gnome-icon-theme moved those icons from hicolor to gnome and the GDM greeter uses no GTK theme so it doesn't use gnome for that. I've opened a bug upstream about that: http://bugzilla.gnome.org/show_bug.cgi?id=330061

I dist-upgraded my flight 4 install today, and now the icons are back and working. (gnome-icon-theme 2.14.1-0ubuntu1, gdm 2.13.0.8-0ubuntu3)

Right, that change fixes the issue:

 gtk+2.0 (2.8.13-1ubuntu1) dapper; urgency=low
 .
   * Sync with Debian
     New upstream version:
     - gtk.Notebook.get_current_page() returns incorrect page number
       when pages hidden (Ubuntu: #30348)
   * debian/control.in:
     - clear the Build-Depends on xlibs-static-pic | xlibs-pic, not required
   * debian/patches/001_fs_documents.patch:
     - use GTK_DEFAULT_FILECHOOSER_DIR by default
     - if GTK_DEFAULT_FILECHOOSER_DIR is not defined use the "Documents" folder
   * debian/patches/001_default_fallback_icon_theme.patch:
     - set the default fallback icon theme to "gnome", fixes the issues with
       the moves of icons from hicolor to gnome
