MySQL configuration not recorded nor taken into account

Binary package hint: gbirthday
I use Ubuntu 11.04 classic, and installed Gbirthday 0.6.4. In Preferences: Database, I chose only "MySQL", then clicked on "Configure" and filled the "Database configuration" form, but there is not even a button to confirm or save that. If I click on the "Save and Close" button of the "Preferences" window without closing the "Database configuration" window, the click is not accepted (the button doesn't give way) and if I close the "Database configuration" window to click on "Save and close", my changes seem lost because, if I try to add a birthday, I get the message: "Could not execute SQLite-query: SELECT id from cal_calendars LIMIT 1; 'NoneType' object has no attribute 'execute'". Note that "cal_calendars is no the name of the table I configure.
I would expect the "Database configuration" have a "Save" button, and, when I add a birthday, that birthday appear in my MySQL database or an understandable error message appear, such as "Gbirthday couldn't connect to your MySQL database called X as user Y using a password.", or "Gbirthday could connect to your MySQL database X but the table 'birthday' was absent.", or "Gbirthday could connect to your MySQL database X but was not allowed to write in the table 'birthday'.".
