I have rechecked for router/tcp congestion causing this problem (e.g. ack
packets not getting through). It seems this will slow down firefox (or any
network application for that matter), but is not the cause of this hang in firefox.

That leaves something in Gecko hanging after the tag has opened but just before
initial rendering starts being the cause... IMHO.

Blake, have you looked into this yet?
