LDAP address book doesn't search LDAP server

Bug Description
When I add an LDAP address book, Evolution won't search the database.
I've entered the following information for the address book:
The "Server" field contains "crtdir.
The above LDAP server answers queries that are performed from the command line; for example:
ldapsearch -x -h crtdir.
will return its search results for the email address "<email address hidden>".
For debugging, I set up an LDAP server on my own computer. According to the LDAP debug log set to max debugging level, evolution doesn't even seem to attempt to connect to the LDAP server.
This bug has been reported in the past, but unfortunately the developers have let the bugs expire:
