I was adding buttons to the Toolbar when it crashed. I tried to add a button which was already there, but still showing as available in the pop-up box. I 'successfully' recreated the crash by doing the same thing.

I don't see any icon in the toolbar that are still in buttons window. I can add/remove buttons without any crash. Can you still reproduce with evince 3.6.0 (ubuntu 12.10)?
