documentviewer crashes whenn adding tools

when I try to add a tool to the symbol's (such as printer or open) documentviewer crasches. After restart, the added tool isn't there where I met it before.

I'm actually not seeing a crash when I add "print" to the toolbar, only for "open" -- are you really seeing a crash if you add the print icon?

sorry it was my fault, the error occurs only when adding the open folder,
