I can confirm that the bug exists on 16.10 too.

Printing from other applications work, like Firefox (pdf.js) and LibreOffice.

This is easy to fix:
Remove the file "~/.config/evince/print-settings" as pointed out in https://wiki.ubuntu.com/DebuggingPrintingProblems

This works, but you have to remove the file every time it is printed.
