Bug Description
Running Ubuntu 18.04 Server virtual machine with UEFI enabled there appears to be graphical issues (pixels in wrong place).
These issues can be temporarily fixed by changing the gfxmode in grub config. But from time to time Ubuntu boots into a machine that has pixels in wrong place. This problem was originally discovered when installing Ubuntu 18 on a xen hypervisor with Secure Boot enabled.
My suspicion is that its a problem with OVMF and Ubuntu. Issues can also be solved by switching to vga=cirrus.

Here is a copy of a working /boot/grub/grub.cfg file that fixed a problem.
When using QEMU, apply some of this config to broken one (UUIDs will not be the same for you!!)
and then when grub menu appears press enter on Ubuntu option to enter non broken machine. If you let grub timeout play out and don't press enter then system will be corrupted.
