existing index file is used even if it is broken

Bug Description
STEPS TO REPRODUCE
1. Open any video file
2. While indexing is in progress (say 50%), shutdown the application. In my case i did a poweroff of the system, but kill should also do it
3. Open the video file again
4. The index file is recycled, but only half of the movie is visible, so it looks as if the video file is broken
EXPECTED
dvbcut detects tha the index is broken and recreates it

Another case is if the video file changed (because it was still recording when indexing the first time)
I know, the index files currently don't have a file header, so comparing to the video file might be difficult. an option is to change the file format, or to check the last mark in the index file and compare it with the end of the video file.
