Missing dm_integrity module makes system fail to boot

Bug Description

If you create a lvm raid set with integrity enabled and it is mounted via /etc/fstab then your system will fail to boot as the dm_integrity module is not included in the initramfs.

Suggested fix is to add `force_load dm-integrity` to the `/usr/share/initramfs-tools/hooks/dmraid` script at the same location as the `force_load dm-mod` and `force_load dm-mirror` location.
