Net install of 16.04 fails when loading squashfs

Bug Description
In trying to get network installs of 16.04 working, I am finding that the installs are failing when attempting to download the filesystem.squashfs over the network.
The URL that is shown in the log is "/cdromhttp://...." instead of just "http://....". Was there a change in the preseed file values that is causing '/cdrom' to be prepended to the URL similar to the change that occurred in 12.10? I could not find anything in release notes about such a change.
