Bug Description

Setup:
VirtualBox 4.2.12 on a Windows 7 64-bit SP1 host
64-bit suacy "mini.iso" built on 2013-05-29

The error message:
"Installation step failed" under "Install the system".
See attached logs for further info.

What I did before it occurred:
I purposely selected to do an expert install in order to learn how more about the installer.
I setup LVM (see in log.)
I chose to download a bunch of extra installer components, including the live installer and the SSH component.
I had virtualbox switch me over from a private network to a bridge to my physical LAN after the installer setup the SSH component. I then reran "Configure the network" to get a new DHCP address.
I chose to do a normal install under "install the system", not a live install.

Other notes:
The first time "Install the System" failed happened at 14:09 UTC. Any subsequent log entries were me retrying steps such as partition the disks and install the system (which failed every other time I ran it).
As seen in one of the screenshots, /dev/saucy-server-vg/ does exist.

Despite what the syslog message says at 14:09:10, /dev/saucy-server-vg was created. Maybe it was created too late.
Also, I misspelled "saucy" earlier.

Also:

1. "Install the system" failed immediately after I selected to do a normal install. Every additional time I ran it, it did the same.

2. I just re-ran the installer and it was completely successful.
The differences this time were that I selected "Command-line expert install" and that I did not download any installer components. I partitioned the disk with the same LVM settings. I selected "Install the system" within 1 or 2 seconds after partitioning the disks succeeded (in case there was a timing issue with /dev/saucy-server-vg/ being detected.)

I can confirm this bug is still present while installing Ubuntu 16.04.2 amd64 (using mini.iso under qemy-system-x86_64). I selected expert command-line install, made all the steps manually, selected a "normal installation" but the installer fails on "Install the system" step. Looking into syslog, for each attempt, I can see:

base-installer: error: Could not find any live images
...
...
...
main-menu[248]: WARNING **: Configuring 'live-installer' failed with error code 1
main-menu[248]: WARNING **: Menu item 'live-installer' failed.
