I would tend to agree with your assessment. The cause of the problems right now seem to be some duplication of the handling of these options between the various involved filters, so that should probably be streamlined.

For reference, I am attaching the PS test page that was used by our customer to exhibit the issues. It is admittedly handcrafted, so it is not representative of what may be generated by an office application.
