3) The title bar contents should change when the application changes it.
4) The change doesn't happen until the window loses focus -- or some other event requires compiz to redraw the titlebar.
I've encountered this behavior in Firefox, GIMP, and GNOME-terminal.
Firefox: Title should change with active tab's contents.
GIMP: When an edited file is saved, the title should lose the "*" prefix
GNOME-terminal: I use a PS1 that changes the terminal title with the working directory

This is a MAJOR usability flaw for me. I've been using Linux since kernel 1.2 (and exclusively for over 10 years). I'm a heavy command line user and rely on the terminal title to accurately report my current working directory. When working in the GIMP, I rely on the title to let me know if I've saved my work or not. All the eye candy is meaningless to me if the basic function of a window manager isn't present.
