Windows disappear without user input and aren't found again on any desks but process still running in background

Bug Description
Was working with a LibreOffice document and all of a sudden, window looked to be minimizing on it's own. However, it is not located on any of the three virtual desks, but is still running as a process. I can continue opening and closing new LO windows. Alt+Tab no longer shows the now "hidden" LO document. This has only been intermittently reproducible infrequently with LibreOffice.
This also happens on a Toshiba Satellite S55-C5247 with Intel integrated graphics, so not correlated to graphics driver (radeon, fglrx, or i915).
In addition, this is not reproducible in gnome-session-
After restarting the application, it happened again, but this time I could go to another virtual disk and click on it, which would cause it to move to another virtual desk. Secondary clicking the window showed it was both minimized and maximized simultaneously, as it noted:

WORKAROUND: If I open a blank document and click "Exit LibreOffice" it offers to save the hidden document.

Windows disappearing, but the process is still running has a notable priority given now I have to unsafely kill the task, risking data loss on the document by killing the process.
