This means something has crashed (probably not apport itself, that's the program that reports crashes): to find out what, look in /var/crash.

I have the same problem, but it started on upgrading from 12.04 Unity 2D to 12.10 full Unity; I attach my crash report. (I know this is probably the wrong way to send one but I am not entering my password at an unidentifiable prompt: see bug 794757).

/var/crash is the folder where crash reports are kept; the name of the report includes the name of the program that crashed, and opening it (with the default "Report a problem...") will show more information and allow you to submit it here. (Do NOT attach the report here as an ordinary file: depending on what crashed, it may contain security-sensitive information.)

Hello Rebecca, just want to thank you for giving me the right info that led
to the success, identifying the right folder, thereby reporting the
problem. My system is now back to normalc. Thank you once again. I'll be in
touch for your assistance any time possible.

This bug has been affecting me for the past two months or more, but until now I haven't been able to find anything useful on the subject.

The notification won't submit a report, though I have tried numerous times. It says the report file is corrupted, then wants me to give my password again.

Thanks for the clue about /var/crash. The crash file has some clues, essentially confirming what I already suspected. The file contains a core dump, which probably explains the issue with file corruption -- the computer has 6 GB main memory.

The crashs appear to be due to pulse audio being invoked from openjdk6. The crash file is named /var/crash/_usr_lib_jvm_java-6-openjdk-amd64_jre_bin_java.0.crash

The first two files mentioned in the crash report are:

/usr/lib/jvm/java-6-openjdk-amd64/jre/bin/java
and
/run/shm/pulse-shm-602413393

These crash reports are no doubt caused by negative interaction between openjdk6 and thinkorswim (a trading platform by TDAmeritrade) because ToS crashes occasionally, but I don't necessarily see a 100% correlation between the two notifications.

Audio signalling by ToS don't guarantee a crash, however. Those most likely to cause a crash appear to be notifications, of which there are several. Opening and closing bells don't generally result in a crash, though I did experience (a couple of months ago) at least one crash apparently caused by the closing bell.

This is a fresh 12.04 install, done in August 2012. I have always kept it up to date.

The system is an HP Pavilion dv6 with quad-core AMD A8 with integral ATI video processor(s), and 6 GB main memory.
