I have to physically unplug my speakers to hear sound ONLY from headphones

Same here: precise amd64, alsa-base 1.0.25+dfsg-0ubuntu, also NVIDIA HDA. If more detail would help, just ask.

It would be nice if someone tried the latest ALSA module to see if the issue still exists there:

sudo apt-add-repository ppa:ubuntu-audio-dev/alsa-daily
sudo apt-get update
sudo apt-get install alsa-hda-dkms

I've installed that ALSA module like you've stated
I will test it later
I don't have any headphones here at the moment
