Bug Description

Ubuntu 12.04
pulseaudio 1:1.1-0ubuntu15

Reproduce Step:
Open "Audio Settings" from the audio icon of indicators.

Expect Result:
There is only Speakers audio output when no headphone is connected.

Actual Result:
There are Speakers and Headphones audio outputs when no headphone is connected.

The problem here is that the Headphone Jack gets an index=1, even though this is not needed.
