Cannot add more than 3 bug entry fields in the ISO Tracker.

Bug Description
Steps to reproduce:
1) Go to the ISO Tracker website http://
2) Start testing
3) If you need more than 3 bug entry fields, clicking on "More bug entry fields" it reloads the page but does nothing, in fact you can't add more than three bug numbers.
Workaround:
Fill in all the three bug entry field, post your result, then click on "More bug entry fields", edit one of the already selected ones and click on "More bug entry fields" again.
