Mouse cursor has inconsistent size and theme

Bug Description
In kubuntu 20.04, the mouse cursor changes size and style depending on which application, or part of application, it is hovering over. It is only sometimes respecting the choice of cursor theme selected in system settings.
As a result, the mouse cursor is sometimes comically small or comically large.
I only noticed this after hooking up an external monitor to my laptop. It might be okay on a single screen, or if all screens have the same size/resolution.
The attachment has 3 pictures, all with the McMojave Cursors theme, size 24, selected from system settings. (The problem is not specific to this theme; it's just an example.)
In the first picture, the cursor has the correct theme and the correct size.
In the second picture, the cursor appears to be from the Breeze cursor theme, but it still has the correct size.
In the third picture, the cursor appears to be from the Breeze cursor theme, and it also has the wrong size.

Also providing the application in use in your pdf example (necessary to know if it's GTK or Qt based) would also be useful.
