The `mkfs.vfat` tool is required to build the image, it is part of the `dosfstools` package (http://packages.ubuntu.com/search?suite=wily&arch=any&searchon=contents&keywords=mkfs.vfat) which may not be installed by default (it wasn't on my system).

The `dosfstools` package should probably be added to the packages installed in the `bootstrap` function.
