XRandR output names should be DVI-0 HDMI-0

pvr1 should be named HDMI and pvr0 should be DVI - much easier to spot which one is which one.

fyi, this is handled properly by:

  https://github.com/robclark/xf86-video-omap

although the new driver is not quite ready for prime-time (need to finish getting DRI2 and EXA working). But should hopefully be working in a month or two.
