Seems to be triggered in journalctl by opening activities and then starting to type something in the search box:

Seems media keys stop working after this crash, and a reboot (or log out/in?) is needed to bring them back.

I confirm this problem.
This crash happens mostly in these cases for me:

1.) Restarting pulseaudio
2.) Suspending computer -> at wakeup the crash happens

This is sometimes accompanied by pixelation of the background on the desktop.

Same problem here. But for me, it happens whenever I open a LibreOffice application.
I'm using Ubuntu GNOME Vivid Vervet.

It started to happen after installing fglrx drivers and reverting back to xorg ATI drivers.

I don't know which log files might be useful, but I can replicate the problem reliably here and I can produce any log needed.
